Patient Group (PPG)

We believe that our patients should be involved in decisions about their care: the how, when and by who they receive it. The only true way we can be sure that the services provided meets the needs of our community is if we hear from you. We would like our healthcare professionals and our patients / service-users to work together, in partnership, to provide the correct care and services to our community.

We are very keen to get a newly established group of invested individuals together and would love to establish a cohort of individuals that represent all aspects of the community we serve. Meaning of REACH

We have worked to devise a patient participation group with a clear identity, purpose and values. The name of this group – REACH is intentional, to represent these values:

R = Representing Community

To be an effective and valued resource to both community and    surgery the group needs to be made up of a fair representation of its patient demographic

E = Engage and Participate

The group is focused on improving patient services – constructively feeding back on current services and having a say in developing new ones

A = Advocate for All

The group is there to represent the community as a whole and not as an individual. It is structured and made up entirely of Surgery patients with Surgery members attending to support

C = Communicate Effectively

Members are respectful and mindful of each other, always adhering to the Constitution and Terms of Reference

H = Harness Diversity

To be an open, welcoming group that embraces diversity to the betterment of all

Purpose of REACH

The purpose of the PPG is intended as a positive – a group in which collaboration takes place to develop the services provided, driven by our patients and provided by our Healthcare professionals. It is an opportunity for Hawthorn Medical Centre to share what health promotions, developments or changes may be happening and for patients to feedback the impact of any care provision.

If you feel that you could represent your community and would like to be involved in what happens at Hawthorn Medical Centre, we would welcome an application. We aim to have a group that truly represents the practice population.

Role of the PPG Committee

  • Represent the community – Act as “a patient voice” giving feedback to both the community and the surgery
  • Work to ensure the right services are available to the local population
  • Provide patients with a platform in which to engage with their Practice – learn about the services and facilities available to them
  • Be involved in the development of new services and changes within the Practice and share updates
  • Help to promote health awareness and patient education
  • Cascade /promote any projects, new services, workshops that are being undertaken within the surgery
  • Gather and evaluate feedback to give to the Practice
